Overview
Pain of the Achilles tendon commonly affects both competitive and recreational athletes, and the sedentary. The largest tendon in the body, the Achilles tendon, endures strain and risks rupture from running, jumping, and sudden acceleration or deceleration. Overuse, vascular diseases, neuropathy, and rheumatologic diseases may cause tendon degeneration. The hallmarks of Achilles tendon problems seem to be damaged, weak, inelastic tissue.
Causes
Common causes of an Achilles tendon rupture include the progression of or the final result of longstanding Achilles tendonitis or an overuse injury. An injury to the ankle or a direct blow to the Achilles tendon. As a result of a fall where an individual lands awkwardly or directly on the ankle. Laceration of the tendon. Weakness of the gastrocnemius or soleus muscles in people with existing Achilles tendonitis places increased stress on the tendon. Steroid use has been linked to tendon weakness. Certain systemic diseases have been associated with tendon weakness. A sudden deceleration or stopping motions that cause an acute traumatic injury of the ankle. Injection of steroids to the involved tendon or the excessive use of steroids has been known to weaken tendons and make them susceptible to rupture. Contraction of the calf muscles while the foot is dorsiflexed (pointed toward the head) and the lower leg is moving forward.
Symptoms
An Achilles tendon rupture is when the tendon that connects the heel bone to the calf muscle tears and the fibers separate. This happens mostly between the ages of 30 and 50, and usually is caused by sports. Symptoms of Achilles tendon rupture include the following. A pop or snap when the tendon tears. Severe pain in back of the ankle, making it nearly impossible to walk. Swelling and discoloration. Tenderness. Inability to rise on toes. A gap in the back of the ankle where the tendons are separated.
Diagnosis
The doctor may look at your walking and observe whether you can stand on tiptoe. She/he may test the tendon using a method called Thompsons test (also known as the calf squeeze test). In this test, you will be asked to lie face down on the examination bench and to bend your knee. The doctor will gently squeeze the calf muscles at the back of your leg, and observe how the ankle moves. If the Achilles tendon is OK, the calf squeeze will make the foot point briefly away from the leg (a movement called plantar flexion). This is quite an accurate test for Achilles tendon rupture. If the diagnosis is uncertain, an ultrasound or MRI scan may help. An Achilles tendon rupture is sometimes difficult to diagnose and can be missed on first assessment. It is important for both doctors and patients to be aware of this and to look carefully for an Achilles tendon rupture if it is suspected.
Non Surgical Treatment
Nonsurgical method is generally undertaken in individuals who are old, inactive, and at high-risk for surgery. Other individuals who should not undergo surgery are those who have a wound infection/ulcer around the heel area. A large group of patients who may not be candidates for surgery include those with diabetes, those with poor blood supply to the foot, patients with nerve problems in the foot, and those who may not comply with rehabilitation. Nonsurgical management involves application of a short leg cast to the affected leg, with the ankle in a slightly flexed position. Maintaining the ankle in this position helps appose the tendons and improves healing. The leg is placed in a cast for six to 10 weeks and no movement of the ankle is allowed. Walking is allowed on the cast after a period of four to six weeks. When the cast is removed, a small heel lift is inserted in the shoe to permit better support for the ankle for an additional two to four weeks. Following this, physical therapy is recommended. The advantages of a nonsurgical approach are no risk of a wound infection or breakdown of skin and no risk of nerve injury. The disadvantages of the nonsurgical approach includes a slightly higher risk of Achilles tendon rupture and the surgery is much more complex if indeed a repair is necessary in future. In addition, the recuperative period after the nonsurgical approach is more prolonged.
Surgical Treatment
Surgery for Achilles tendon rupture requires an operation to open the skin and physically suture (sew) the ends of the tendon back together, has a lower incidence of re-rupture than nonsurgical treatment. Allows return to pre-injury activities sooner and at a higher level of functioning with less shrinkage of muscle. Risks are associated with surgery, anesthesia, infection, skin breakdown, scarring, bleeding, accidental nerve injury, higher cost, and blood clots in the leg are possible after surgery. Surgery has been the treatment of choice for the competitive athlete or those with a high level of physical activity, for those with a delay in treatment or diagnosis, and for those whose tendons have ruptured again.
Prevention
To help prevent an Achilles tendon injury, it is a good practice to perform stretching and warm-up exercises before any participating in any activities. Gradually increase the intensity and length of time of activity. Muscle conditioning may help to strengthen the muscles in the body.
